Who she is
Dr. Donna D’Alessio is board-certified in Physical Medicine & Rehabilitation and was Chief Resident in Physical Medicine & Rehabilitation at Mount Sinai Hospital. She leads Montville Pain & Veins in Towaco, New Jersey, and has performed thousands of vein procedures.
Why Physical Medicine & Rehabilitation matters here
PM&R is not a well-known specialty outside medicine, and it is the reason this practice works the way it does.
It is the discipline concerned with restoring function without operating. A PM&R physician’s training is in working out what a body has stopped being able to do, why, and what sequence of interventions gets it back — rather than in deciding what to cut out. Injections, rehabilitation, load management and the mechanics of how somebody moves are the native tools of the specialty, not adjuncts to a surgical practice.
That orientation is why the plan you leave with here tends to have more than one component, and why “you need surgery” is something you will be told honestly rather than steered towards.
The vein half
Her training in physical medicine also explains how she approaches veins. The complaint people bring — heavy, aching legs, swollen ankles, visible veins — is a functional one, and the question is what the leg has stopped doing properly. That is answered with a duplex ultrasound map of the venous system before anything is treated, because the visible vein is very often not the problem.
She has performed thousands of vein procedures and does not perform vein stripping, which has been superseded by catheter and foam techniques done in the office under local anaesthetic.
How she works
Patients describe the same thing repeatedly: that she listens, that she explains, and that she treats the person in front of her rather than a category. Whether you stand all day, carry small children, travel constantly for work, or are trying to get back to a sport you have given up on — that is the context in which a treatment plan either fits your life or does not.
She will also tell you when the answer is that you do not need a procedure. That is a less profitable sentence and a more useful one.
