After the trail, on the stairs
The southern end of Ramapo Mountain State Forest reaches into the north of Pompton Lakes, and the forest’s Cannonball Trail starts inside the borough. If a walk there now ends with a sore knee, or the stairs at home are taken one at a time, it is worth knowing which part of the knee is complaining. One pattern is especially common: pain at the front of the knee, around or behind the kneecap, worse going downstairs and after sitting for a long time. That is patellofemoral pain, and it is often as much about the thigh and hip as about the knee.
Getting here
From Pompton Lakes, take the Paterson-Hamburg Turnpike and the Newark-Pompton Turnpike to Jacksonville Road and on into Towaco — about nine miles to 624 Main Road, Suite 2B, close to fifteen minutes without traffic, with parking on site.
The hip belongs in a knee examination
How the kneecap moves depends on how the quadriceps and the hip muscles pull on it, and weakness at the hip is a frequent contributor to front-of-knee pain. A knee looked at in isolation misses that. So Dr. Donna D’Alessio, board certified in Physical Medicine & Rehabilitation, watches you walk, follows how the kneecap tracks, presses on the exact structure that hurts, tests the ligaments and menisci, and measures the strength of the quadriceps and gluteal muscles. Imaging is ordered when it would answer a particular question the examination raises, not as a routine first step.
What changes the knee
- Strength. Stronger thigh and hip muscles change how the kneecap is pulled and how much load the joint takes with every step. It is the part most under your control, and the part that lasts.
- Load. Managing how much the knee is asked to do while it settles is part of the treatment, not a rest cure.
- A joint injection where an arthritic or irritated joint is inflamed — about fifteen minutes, and a window in which the strengthening becomes possible.
- PRP for mild to moderate arthritis and certain tendon problems, prepared from a sample of your own blood. It is generally paid for out of pocket, and you hear the price first.
Pompton Lakes patients ask
Is pain at the front of my knee the start of arthritis? Not necessarily. Arthritis after middle age brings morning stiffness and aching after activity; kneecap pain is more often about how the muscles are pulling, and the work to fix it is usually in the thigh and hip. The examination tells the two apart.
How soon would an injection help? If a local anaesthetic is included, relief can be immediate and fade within hours — a useful sign that the joint is the source. The steroid then builds over several days and peaks over a week or two.
I was told the knee is bone on bone. Is PRP still worth trying? Probably not, and you will be told so. PRP does not regrow cartilage that has gone; its evidence is in mild and moderate disease.

Tell us what’s going on
A consultation is a conversation and an examination, not a sales appointment. Dr. D’Alessio will tell you what she thinks is causing it, what can be done about it, and — when that is the honest answer — that you do not need a procedure.
