In the first few weeks

Sciatica is sharp, burning or electric pain running from the low back or buttock down the back of one leg, sometimes with pins and needles toward the foot. A large share of episodes ease substantially within six to twelve weeks. While yours does, keep moving within what the pain allows. Days in bed weaken the muscles that support the spine and stiffen everything around them, so short, frequent bouts of gentle movement — and not staying in one position for long — are the usual advice until someone has examined you.

When to stop waiting

Book an assessment when the pain is:

  • severe, at any stage
  • no better after six to twelve weeks
  • coming back, episode after episode
  • bringing weakness with it

Go to an emergency department at once, without waiting for an appointment, for new weakness in a leg or foot, numbness around the groin, inner thighs or buttocks, or any change in bladder or bowel control. Those can signal cauda equina syndrome, which is rare and cannot wait.

The drive in

Montville Pain & Veins is at 624 Main Road, Suite 2B, Towaco. From the middle of Pompton Lakes it is about nine miles — the Paterson-Hamburg and Newark-Pompton Turnpikes, then Jacksonville Road — and close to fifteen minutes without traffic. If sitting in the car is the worst part of your day, say so at the visit; how the pain behaves when you sit is one of the best clues to its cause.

What the visit sorts out

“Sciatica” names a symptom rather than its cause. The nerve can be irritated by a disc pressing on a root in the spine, a narrowed spinal canal, the piriformis muscle deep in the buttock, or the sacroiliac joint referring pain into the leg. Dr. Donna D’Alessio, board certified in Physical Medicine & Rehabilitation, works out which from your account of the pain and from testing reflexes, strength, sensation and the straight-leg raise, with the spine, the sacroiliac joints and the buttock checked by hand. Any MRI you already have is weighed against what she finds.

The treatment then follows the cause: a trigger point injection for a piriformis locked in spasm, a joint injection where a joint is the source, PRP where tissue needs help to heal, and in every case a rehabilitation plan for the mechanics underneath. Nothing here involves an operation, and if a surgeon is what you need, you will hear that and be referred.

Pompton Lakes patients ask

Is sciatica the same thing as a slipped disc? No. A disc is one cause among several, and disc bulges turn up on the scans of many people with no pain at all — so a bulge is matched against your examination before it is blamed.

Does an injection into the buttock muscle hurt? Briefly. The needle is fine; most people feel a deep cramp or notice a quick twitch as the muscle responds, which is a good sign, and the spot can be sore for a day or two.

A consultation is a conversation and an examination, not a sales appointment. Dr. D’Alessio will tell you what she thinks is causing it, what can be done about it, and — when that is the honest answer — that you do not need a procedure.
